Clinical Analysis of the Benign Gastric Tumors

Baek Kwi-Soon
Abstract
Benign gastric tumors ara rare. Since the advent of gastrofiberscopy, the frequency of the diagnosis of benign gastric tumors has increased and their management has become more important, 54 cases of benign gastric tumors which had been treated
at
the
Soo Chun Hyang University Hospital during 7 years from June 1983 through May 1990 were analysed.
@ES The results were as follows:
@EN 1) The incidence of benign gastric-tumors was 8% of all gastric tumors in this period. 64.8% of the benign gastric tumors were epithelial polyps, 20.4% were mesenchymal neoplasm and 14.9% were miscellaneous tumors with inflammatory fibroid
polyposa,
heterotopic pancreas, hamartoma.
2) 68.5% of all benign tumors occured in females. A female. A female to male ratio was 2.18 to 1. The age range was 23 to 73 years and the peak incidence was between 5th decade(27.8%) and 6th decade (31.5%).
3) The most common presenting symptom was epigastric pain, occuring in 70.3% of patients, 20.4% of the patients had no symptoms.
4) preoperative endoscopic biopsy was performed in 53 patients for diagnosis, and UGI series in 19 patients, endoscopic ultrasonogram in 3 patients and abdominal CT scan in 1 patient.
5) 53.6% of tumor was located in antrum of the stomach, 36.2% in the body and 10.1% in the cardia.
6) The size of the tumor ranged from 4 mm to 100 mm in diameter(mean diameter; 12.0 mm). 36.2% of the tumor was less than 5 mm in diameter. 17% of the patients had the multiple lesions.
7) Endoscopic polypectomy and laparotomy were performed in 33 patients(61%) and 22 Patients(40%) respectively. Wedge resection underwent in 12 patients, gastrectomy in 7 patients and polypectomy with gastrotomy in 3 patients. Of 22 patients
undergoing
laparotomy, 10 patient had leiomyoma, 7 Patients hyperplastic polyp, 2 patients inflammatory polyp, 1 patient adenomatous polyp, 1 patient neurilemmoma and 1 patient gastritis cystica polyposa.
8) Of 22 patients undergoing laparotomy, 7 patients were not performed frozen biopsy while the surgery. Among them, one patient was revealed the malignancy in histology.
These results suggest that, when surgical treatment was needed for gastric tumor, frozen section at the time of operation is essential and treatment should be dictated by the pathology.
